
Breast Augmentation
No exercise, hormone or medication meaningfully changes the size of a small breast. Augmentation does — with an implant placed, in most cases, behind the pectoral muscle through a short incision.

How it is performed
The implant is inserted through a small incision and, in most cases, placed behind the pectoralis major muscle. The texture of modern implants is practically indistinguishable from breast tissue. How much size can be added is limited by how far the existing tissue will comfortably stretch: a breast that was larger before pregnancy adjusts more easily than one where little breast tissue ever developed.
There are several possible incision sites — around the lower border of the areola (leaving a scar of about 2.5 cm that fades to near invisibility), under the breast fold, or through a small incision in the armpit — and several implant types, including cohesive silicone gel and saline. Each has advantages in particular situations. The final choice of incision and implant is made together with you, based on Dr. Öksar's experience and your anatomy.
Size is estimated before surgery with pre-formed sizers at consultation, based on your stature, chest measurements and existing breast tissue. Bigger is not automatically better: an oversized implant in a small pocket produces tension, poor contour and palpable firmness. The aim is a soft, natural-looking breast.
Who is it for
- Breasts that have never developed fully, or that have shrunk after one or more pregnancies.
- Sagging with modest volume loss after pregnancy, where fullness rather than lifting is the priority.
- Asymmetry or deformity following other breast surgery.
- Selected reconstructive cases after breast removal, where the plan must never compromise cancer treatment or recovery.

Recovery timeline
- Day 1
One night in hospital. Tightness across the chest is normal as the muscle accommodates the implant.
- Days 2–3
Most patients return to normal daily activity, avoiding lifting.
- Week 1
Recovery is largely complete for desk work; a support bra is worn as instructed.
- Weeks 2–3
Strenuous exercise is avoided for about three weeks.
- Months 2–3
Implants settle and most patients stop being aware of them.
Risks & honest expectations
- Capsular contracture: the natural fibrous capsule around the implant can tighten and firm the breast. Sub-muscular placement and careful early mobilisation reduce the risk; if significant, a secondary release procedure may be needed.
- Unfavourable scarring, more likely with the inframammary incision than the areolar one.
- Collection of blood around the implant in the first 24–48 hours, occasionally requiring re-opening of the incision.
- Infection at the implant site is unusual and can often be treated without removing the implant.
- Temporary change or loss of nipple sensation, usually returning within three to six months.
- Rippling of the skin over the implant, and minor asymmetry — small differences are within normal limits.

Questions about this procedure
Do implants affect breast-feeding or cancer risk?
There has never been a demonstrated relationship between silicone implants and the later development of breast cancer, and implants do not interfere with future breast-feeding. There is no evidence of health risk to the child.
Do implants need to be replaced?
Modern implants rarely leak or rupture. If it happens, the shape or feel of the breast changes and the implant is replaced. There is no fixed expiry date, but long-term follow-up is sensible.
Above or below the muscle?
Sub-muscular placement is used in most cases because it lowers the rate of capsular contracture and gives a more natural upper slope, particularly in slim patients.
